Referrals will be considered for individuals from across Hampshire, Dorset, Wiltshire, Berkshire, Isle of Wight, Surrey and West Sussex.
Naomi House accepts referrals for children with life limiting or life threatening conditions from 0-16 years of age from across the central south of England, including the counties of Berkshire, Dorset, Hampshire, Isle of Wight, Surrey, West Sussex and Wiltshire. To meet our criteria for Naomi House, it should be considered unlikely that the referred child will reach 18 years of age.
Jacksplace accepts referrals for young people with life limiting or life threatening conditions from 16 years of age from across the same area. Referrals for Jacksplace can be made from outside our area, but we will seek statutory funding to support the care of these young adults. To meet our criteria for Jacksplace, it should be considered unlikely that the referred young person will live beyond 35 years of age.
